Please read and understand the Terms of Membership:

There is a one-time enrollment fee of $50 (individual or family).
Same day visits are not guarantee if call after 12pm.
​Membership is an annual contract. You may cancel at any time with a 30 day written notice and a reason for canceling.
​Re-enrollment fee of $120 will be applicable if members cancel then re-enroll at a later date within the same year.
There are no restrictions on frequency, type or duration of office visits per month.
Costs of medications, immunizations and some procedures not included.
Membership pricing is subject to change in the future.
Convenient monthly billing through automatic bank withdrawal or credit card payment.
Three month minimum enrollment, then cancel anytime with 30 day notice.
​Wellspring Medical Care Plans are NOT insurance and should not be a replacement for a health insurance plan. We advise our patients to have insurance to cover catastrophic illnesses or more advanced diagnostic procedures and specialist’s care. 
​We recommend pairing a high deductible insurance plan with our direct primary care plan for a cost effective medical care.*
Our DPC Membership plans are only valid in Wellspring Medical Care Clinic.
