1. Introduction Welcome to Wellspring Medical Care PLLC. By accessing and using our website, services, and facilities, you agree to comply with the following terms and conditions. Please read these terms carefully before using our services.
2. Services Provided Wellspring Medical Care PLLC offers primary care services, including but not limited to preventive care, chronic disease management, and urgent care. In case of emergencies, patients will be referred to the nearest major hospital after receiving basic first aid and medical evaluation. Incase of emergency always call 911.
3. Direct Primary Care (DPC) Membership a. Membership in the DPC program provides access to primary care services for a fixed monthly or annual fee. b. DPC membership does not constitute health insurance and should not be considered a substitute for emergency or specialized medical care. c. Membership fees must be paid as per the agreed-upon schedule. Failure to make timely payments may result in the suspension or termination of membership benefits. d. Membership covers routine visits, consultations, and select medical procedures. Any additional tests, specialist referrals, or hospital services may incur separate charges. e. Patients have the right to cancel their membership with a 30-day written notice. Refund policies, if applicable, will be outlined in the membership agreement. f. Wellspring Medical Care PLLC complies with all applicable Texas laws governing direct primary care agreements. Wellspring Medical Care PLLC reserves the right to cancel the membership immediately without a refund if a patient is found to be abusing the membership, including but not limited to excessive or fraudulent use of services.
4. Use of Electronic Medical Records (EMR) a. Wellspring Medical Care PLLC utilizes an advanced Electronic Medical Records (EMR) system to maintain patient data securely and efficiently. b. By using our services, patients consent to the electronic storage, retrieval, and transmission of their medical information in compliance with applicable data protection laws. c. Patient data will only be shared with authorized healthcare providers, insurers, or legal authorities as required by law or with explicit patient consent. d. Patients have the right to request corrections to inaccuracies in their medical records as per Texas medical regulations.
5. Appointment Scheduling and Cancellations a. Patients are encouraged to schedule appointments in advance to ensure timely access to care. b. Cancellations or rescheduling must be done at least 24 hours before the appointment time. c. Missed appointments without prior notice may be subject to a cancellation fee. d. Patients who arrive more than 15 minutes late for an appointment may need to reschedule.
